New Pupil Starter Pack

Thank you for applying to Little Bowden School

If you are applying to school for the first time, you will see that there are a few forms to complete and these will need to be handed back in to school with your child’s birth certificate (which will be photocopied and returned) by the deadline given.

If you are applying mid-school year (Years 6 down to Rec) then you will not need to complete them all.

Below is a brief resume of the forms in this section and whether you should complete them or not.  If you have any questions please contact admin@littlebowden.leics.sch.uk and we will do our best to help you.

1.Parent Guide to Early Years – information about what to expect when your child starts in the Early Years Foundation Stage (EYFS) – for reading only.

 

2.Starting School – Nursery World article on children starting school – for reading only.

 

3.Acceptable use of the Internet – Expectations of internet usage as well as permission from parents/guardians to allow internet use during lessons – to be completed by all applicants.

 

4.Photographs & Use of Images – Conditions of using photographic, video or website images of your children - to be completed by all applicants.

 

5.Parental consent for ‘no transport’ visits – When children leave the school premises on a visit or walk to somewhere local (including swimming) we need parental permission for them to leave school -  to be completed by all applicants.

9.a) Changing underwear – In the event of children having an accident we ask permission for your child to be changed as necessary – Reception (new starters) only, to complete this form.

9.b) Baseline Assessments – It is a statutory requirement that this questionnaire is completed -        Reception (new starters) only, to complete this form.

9.c)Transition Questionnaire – This questionnaire helps the teachers of new starters in Reception an idea of your child and how they are coping with the idea of starting school –

10. The Official Application Form – This is the most important form of all, all of the information given on this form will be entered in to the school data base.

It is vital that all contact information is completed and that emails and telephone numbers are written clearly.

New starters in Reception will also need to bring a copy of their birth certificate – this is a legal requirement. It will be photocopied and returned to you asap.

ALL applicants, including ‘mid-term transfers’ must complete this form .                                                                   

                                                                                                                                                                               

11.Inhaler use – This is if your child has symptoms of asthma or has been diagnosed with asthma. Please read, if you feel that this is relevant to your child then this must be completed and returned.

 

12. Diet/Allergies – Like the inhalers, this is if your child has any dietary issues or allergies that we should be aware of. Please read, if you feel that this is relevant to your child then this must be completed and returned.

 

13. Medical/AllergiesAs above, please read, if you feel that this is relevant to your child then this must be completed and returned.
